Additional Patient Information

Please provide us with a 24 to 48 hours notice if you need to cancel your appointment.

Once you are deemed eligible for medical services, an appointment will be scheduled with a case manager and a new patient appointment with a medical provider will follow.

If you are applying for dental services, your name will be added to our dental services working list and you will be contacted when an appointment becomes available.

Eligibility to remain a patient is re-determined annually.

If you are not eligible for our services, other resources may be available to you. Our case management team will be happy to help you access other services and enroll in benefit programs if you qualify.
